Sublingual Sedation Debated

Is it time to consider sublingual sedation for cataract surgery? Two ophthalmic surgeons debated that point recently.

John P. Berdahl, MD, who practices at Vance Thompson Vision in Sioux Falls, SD, said his group has used the so-called MKO Melt in more than 10,000 cases. “It has become our standard approach,” he noted, preferred by patients and nurses. He cited a survey that shows three-fourths of patients prefer it, and 85% say that the anesthesia effect is “just right.” The key to success is preoperative evaluation.

Meanwhile, Kathryn M. Hatch, MD, refractive surgery service site director, Massachusetts Eye and Ear in Waltham, said she prefers to individualize each patient’s anesthesia plan. “Using one type of anesthesia for all patients who differ in size, medical comorbidities, and levels of anxiety is not feasible during cataract surgery.” Timing on onset will vary, meaning the MKO Melt’s effect might be lost when it is most needed.
