Phacoemulsification With Excisional Goniotomy Reduces IOP
Phacoemulsification with excisional goniotomy using the Kahook Dual Blade (New World Medical, Rancho Cucamonga, CA) reduces intraocular pressure (IOP), according to a recent analysis presented at the 2019 American Academy of Ophthalmology meeting. It is the longest follow-up study of the combined procedures.
The prospective, multi-center case series included 26 participants with cataracts and glaucoma undergoing phacoemulsification with excisional goniotomy. IOP and number of IOP medications used were recorded preoperatively and through 3 years follow-up.
Results showed a 31% mean reduction of IOP (baseline: 18.9 mmHg, last follow-up: 13.0 mmHg) and 70% mean reduction of medications used (baseline: 1.8 medications, last follow-up: 0.5 medications). Overall, 69.2% of participants experienced a 20% reduction in IOP from baseline, and 84.6% were using at least 1 less medication.
Using excisional goniotomy with phacoemulsification provides effective long-term reductions in IOP and IOP medications. Data collection and analysis are ongoing.
