10-year outcomes of first-generation iStents with cataract surgery show sustained IOP reduction

Combining 2 first-generation iStents with cataract surgery provides effective and sustained intraocular pressure (IOP) reduction over 10 years in patients with primary open-angle glaucoma (POAG), according to a study presented at AAO 2024.

The longitudinal case series followed 56 eyes with varying severity of POAG for up to 10 years.

The study reported an 82% surgical success rate, with only 10 eyes requiring additional glaucoma surgery.

 

Intraocular pressure (IOP) significantly dropped by 27%, from an average of 18.9 mmHg preoperatively to 13.8 mmHg after 10 years (P < 0.001). Although patients initially reduced their use of antiglaucoma medications following surgery, medication needs gradually returned to near-baseline levels after 7 years. 

 

The safety profile of the procedure was favorable, with stable retinal nerve fiber layer thickness observed over the follow-up period. Researchers concluded that the combined approach of cataract surgery with 2 iStents is an effective and safe option, providing long-term IOP reduction and retinal health preservation for surgery-naïve POAG patients.
