Study compares Tanito microhook trabeculotomy to iStent Inject for glaucoma surgery

In the comparison of 2 surgical procedures, Tanito microhook trabeculotomy (TMH-CE) and iStent inject W (Inject-CE), performed in conjunction with cataract surgery for glaucoma patients, TMH-CE demonstrated a more significant reduction in intraocular pressure (IOP) and the number of medications compared to Inject-CE. However, Inject-CE showed a quicker recovery in best-corrected visual acuity.

The study, which included 78 eyes from 39 participants, evaluated key metrics such as intraocular pressure (IOP), the number of antiglaucoma medications, best-corrected visual acuity, anterior chamber flare. and corneal endothelial cell density preoperatively and at various postoperative intervals.

The preoperative IOP was significantly higher in the TMH-CE group (19.6 ± 6.7 mmHg) compared to the Inject-CE group (15.7 ± 3.8 mmHg). At the 12-month follow-up, TMH-CE demonstrated more substantial reductions in IOP and medication numbers (6.6 mmHg, 27.6% and -1.1, respectively) compared to Inject-CE (2.7 mmHg, 12.4% and -0.7, respectively). However, IOP and medication-number levels were statistically identical between the groups at this juncture.

The TMH-CE group exhibited higher anterior chamber flare, a greater frequency of layered hyphema, and a higher score for anterior chamber floating red blood cells in the early postoperative periods. Despite these differences, later follow-up assessments revealed equivalent changes in BCVA, ACF, and corneal endothelial cell density between the groups.
