Ellipsoid zone as a meaningful endpoint in AMD

Arshad Khanani, MD, of Sierra Eye Associates, spoke with Ophthalmology 360 about a study he presented at the AAO 2025 Annual Meeting, which focused on the use of the ellipsoid zone as a clinically relevant endpoint of age-related macular degeneration (AMD).

Hi, I’m Arshad Khanani from Sierra Eye Associates in Reno, Nevada. At AAO 2025, on behalf of my co-authors, I presented the data from the global consensus from the OCT to function, which we call OCT-Fx Delphi study, looking at EZ as a clinical trial endpoint in AMD.

The purpose of this study is aimed to establish global consensus on the role of ellipsoid zone, or EZ, as a clinically relevant endpoint connecting structure and function in AMD trials. Looking at the results across 2 rounds of surveys, consensus was actually reached on multiple things. EZ loss correlating with BCVA microperimetry. EZ loss can be used for inclusion-exclusion criteria in AMD trials, and of course, looking at geographic atrophy, suitable endpoints include EZ loss, and EZ to RPE thickness as a primary endpoint, and EZ to RPE area as a secondary endpoint. I think this is really important work for the field because more and more trials are looking at ellipsoid zone as a primary endpoint and having this consensus, which was developed using a steering committee and global experts who are participating in this Delphi panel is really meaningful for the field.

In conclusion, I think that these findings support that EZ is a meaningful endpoint that may guide clinical trial design and improve how we connect OCT anatomy for patient function. At the end of the day, we will continue to collect this data and we’ll be presenting this data at future meetings. I think what we want to achieve is that this Delphi panel will really guide the clinical trial design for future trials and really make the community aware of using EZ as an endpoint in AMD trials.
