Enhanced uveoscleral outflow with scleral allograft shows promising two-year results

Scleral allograft reinforcement of an ab-interno cyclodialysis provides a safe, durable, and effective method for lowering intraocular pressure (IOP) by enhancing uveoscleral outflow, with sustained results at 24 months, according to a poster presented at AGS 2025. This minimally invasive technique, performed alongside cataract surgery, shows promise as a long-term bio-scaffold solution comparable to MIGS procedures.

This technique utilizes a homologous scleral allograft, precision-trephined bio-tissue, to reinforce an interventional cyclodialysis, providing structural support for enhanced aqueous outflow.

In the 24-month study including 31 patients the allograft showed excellent biocompatibility, with no signs of persistent inflammation or migration. Optical coherence tomography confirmed stable positioning and seamless integration with native tissues. After 2 years, patients experienced a 41% average reduction in IOP, along with a decreased need for IOP-lowering medications.
