AS-OCT shown to be a reliable alternative to gonioscopy for angle triage

Ophthalmology 360

Specialist-reviewed anterior segment optical coherence tomography (AS-OCT) can safely and effectively replace gonioscopy as a triage tool for angle assessment in virtual glaucoma clinics, according to a study.

Researchers at Sheffield Teaching Hospital evaluated 384 eyes referred to their glaucoma service over a 5-month period, using both AS-OCT and gonioscopy to determine whether angles were open or closed. Trained ophthalmic science practitioners performed gonioscopy, and 2 glaucoma subspecialists independently reviewed AS-OCT images.

Of the 358 eyes analyzed, AS-OCT demonstrated a high sensitivity (97.1%) and strong positive predictive value (95.7%) for identifying open angles, though specificity was lower (36.2%). The researchers concluded that AS-OCT, when interpreted by glaucoma specialists, is a safe and effective triage method.
