Best of 2025: Optimizing anti-VEGF strategies in DME
Our most-read retinal coverage of 2025 highlights how higher-dose therapies and real-world evidence are refining treatment approaches for AMD and diabetic macular edema. From registry and global study data informing extended dosing intervals with aflibercept 8 mg to insights on the timing of anti-VEGF therapy around cataract surgery, these articles reflect continued progress toward more durable and individualized retinal care.
IRIS Registry data shows early aflibercept 8 mg use allows for 12-week injection intervals in AMD and DME cohorts
Diana Do, MD, of Stanford University, spoke with Ophthalmology 360 about a study that used IRIS Registry data to determine whether use of aflibercept 8 mg at the beginning of treatment for AMD or DME could improve outcomes and interval dosing.

Real-world study offers early insights into aflibercept 8 mg for DME
Early real-world findings from the global SPECTRUM study suggest that aflibercept 8 mg is being effectively used in both treatment-naïve and previously treated patients with diabetic macular edema, with enrollment spanning 12 countries. Interim data highlight distinct baseline characteristics between cohorts, offering an early snapshot of how this higher-dose therapy is being applied across diverse DME populations in routine clinical practice.

Preoperative anti-VEGF therapy reduces swelling after cataract surgery in diabetic eye
Giving anti-VEGF injections 1 week before cataract surgery may better protect against postoperative macular swelling in patients with diabetic macular edema than treating at the time of surgery, according to a study. Among 4 diabetic patient groups, only those who received preoperative anti-VEGF avoided a significant postoperative increase in central macular thickness, with more stable macular measurements after surgery, highlighting the potential benefit of earlier intervention in DME patients undergoing cataract extraction.
